Transition of Frail Elderly from acute to community care

The Thompson Division of Family Practice focuses on improving the transition of the frail elderly from acute care to residential and community settings with the inclusion of a robust quality improvement process that focuses on the reduction of unnecessary medications, or Polypharmacy.  This proposal focuses on a request to increase funding to support the integration and inclusion of the SCC Polypharmacy work within a transitions project.
